Our Adult Case Management Team coordinates many services for our enrolled clients, including in-home and day support services, foster and group home referrals, transportation and employment supports.
We assist eligible adults who reside in a variety of settings, including individuals living independently in their own homes, people living with their family or friends, and adults receiving residential services.

An Individual Support Plan ISP is crafted with each enrolled individual to identify supports based upon their health and safety needs, interests, choices and goals. The Service Coordinator works closely with the individual and their support network to help them develop and progress towards their identified goals including aspirations, community integration and employment. Types of support services include attendant care, relief care, chore services, environmental modifications, and behavior supports.
Residential services delivered in neighborhood foster or group homes to people who require hour care, supervision, support, and training. Day Support programs provide support to encourage and engage people in activities in a variety of community settings. Employment programs help people learn skills and gain experience to access community-based jobs.
